It looks like Floris found another one! Based on a clue from Floris, I found another one of these. This one is also from Bright Bricks, but it was a little more difficult to obtain proof that it is bright Bricks as I could not find anything conclusive other than the fact that Bright Bricks also has a few pictures of this ambulance on their gallery page.
Here is the set from a Twitter post in 2014. From what I can gather, it appears that they held a drawing and gave these to winners of the drawing. Here are some details about the drawing. I have no idea how many of these were made in total and I have never seen one for sale on eBay.
