This book was give to approximately 125 – 150 people at the 2010 New York Toy Fair along with a White Boba Fett in a plain clear bag..  I believe that they were also handed out at the Lego store in New York or at en event in the UK.  I cannot seem to remember exactly where.  But I believe that about 2000 of these books were created.  I scanned in all of the individual pages below.  I just looked on eBay and they have 2 currently available here and here.
